For New Appointments:

We strive to see our patients promptly at their scheduled appointment time. Please assist us in keeping our schedule running on time by following these simple guidelines:

  • When making your appointment, clearly state the reason for your visit  in order to guarantee enough time with the care team.

  • If you arrive late for your appointment, you may be asked to reschedule.

  • Please consider arriving at least 15 minutes prior to your appointment time to complete intake forms and to review your current information. 

Our intake forms are LIVE!  No more being asked to download documents, print, fill out, and return to the physician.  Remix Medical is able to send you secure intake forms to your email or cell phone.  Now you or your family can fill out all forms and provide all insurance information prior to any visit - SAVING you from frantically filling out the paper work during the visit and allowing you to spend more time with your care team.

Please bring the following items with you to every appointment:

  • Photo ID such as driver's license or state-issued ID

  • Insurance card(s) and information

  • Payment via MasterCard, Visa, American Express, or personal check

  • Please have available a complete list of medications you are currently taking, including any over-the-counter medications, vitamins or supplements.   

  • Complete list of other physicians you see, their contact information, and why you see them

  • Results from any tests conducted by other health care workers outside of Remix Medical since the last visit, including any progress notes.
